NOTE REGARDING MAILING OF YOUR PASSPORT(S)

Passport Services will not mail a U.S. passport to a private address outside the United States or Canada. If you do not live at the address listed in the "mailing address", then you must put the name of the person and mark it as "In Care Of" in item # 8. If your mailing address changes prior to receipt of your new passport, please contact the National Passport Information Center.

If you choose to provide your email address in Item #6 on this application, Passport Services may use that information to contact you in the event there is a problem with your application or if you need to provide information to us.

You may receive your newly issued passport book and/or card and your returned citizenship evidence in two separate mailings. If you are applying for both a U.S. passport book and passport card, you may receive three separate mailings; one with your returned citizenship evidence, one with your newly issued passport book, and one with your newly issued passport card.

FEDERAL TAX LAW

Section 6039E of the Internal Revenue Code (26 U.S.C. 6039E) and 22 U.S.C 2714a(f) require you to provide your Social Security number (SSN), if you have one, when you apply for or renew a U.S. passport. If you have never been issued a SSN, you must enter zeros in box #5 of this form. If you are residing abroad, you must also provide the name of the foreign country in which you are residing. The U.S. Department of State must provide your SSN and foreign residence information to the U.S. Department of the Treasury. If you fail to provide the information, your application may be denied and you are subject to a $500 penalty enforced by the IRS. All questions on this matter should be referred to the nearest IRS office.

NOTICE TO CUSTOMERS APPLYING OUTSIDE A DEPARTMENT OF STATE FACILITY

If you send us a check, it will be converted into an electronic funds transfer (EFT). This means we will copy your check and use the account information on it to electronically debit your account for the amount of the check. The debit from your account will usually occur within 24 hours and will be shown on your regular account statement.

You will not receive your original check back. We will destroy your original check, but we will keep the copy of it. If the EFT cannot be processed for technical reasons, you authorize us to process the copy in place of your original check. If the EFT cannot be completed because of insufficient funds, we may try to make the transfer up to two times, and we will charge you a one-time fee of $25, which we will also collect by EFT.

FEE REMITTANCE

Passport service fees are established by law and regulation (see 22 U.S.C. 214, 22 C.F.R. 22.1, and 22 C.F.R. 51.50-56), and are collected at the time you apply for the passport service. If the Department fails to receive full payment of the applicable fees because, for example, your check is returned for any reason or you dispute a passport fee charge to your credit card, the U.S. Department of State will take action to collect the delinquent fees from you under 22 C.F.R. Part 34, and the Federal Claims Collection Standards (see 31 C.F.R. Parts 900-904). In accordance with the Debt Collection Improvement Act (Pub.L. 104-134), if the fees remain unpaid after 180 days and no repayment arrangements have been made, the Department will refer the debt to the U.S. Department of Treasury for collection. Debt collection procedures used by U.S. Department of Treasury may include referral of the debt to private collection agencies, reporting of the debt to credit bureaus, garnishment of private wages and administrative offset of the debt by reducing, or withholding eligible federal payments (e.g., tax refunds, social security payments, federal retirement, etc.) by the amount of your debt, including any interest penalties or other costs incurred. In addition, non-payment of passport fees may result in the invalidation of your passport. An invalidated passport cannot be used for travel.

USE OF SOCIAL SECURITY NUMBER

Your Social Security number will be provided to U.S. Department of Treasury, used in connection with debt collection and checked against lists of persons ineligible or potentially ineligible to receive a U.S. passport, among other authorized uses.

NOTICE TO APPLICANTS FOR OFFICIAL, DIPLOMATIC, OR NO-FEE PASSPORTS

You may use this application if you meet all of the provisions listed on Instruction Page 2; however, you must CONSULT YOUR SPONSORING AGENCY FOR INSTRUCTIONS ON PROPER ROUTING PROCEDURES BEFORE FORWARDING THIS APPLICATION. Your completed passport will be released to your sponsoring agency for forwarding to you.

PROTECT YOURSELF AGAINST IDENTITY THEFT!

REPORT YOUR LOST OR STOLEN PASSPORT BOOK OR PASSPORT CARD!

For more information regarding reporting a lost or stolen U.S. passport book or passport card (Form DS-64), or to determine your eligibility for a passport renewal (Form DS-82), call NPIC at 1-877-487-2778 or visit travel.state.gov.

NOTICE TO U.S. PASSPORT CARD APPLICANTS

The maximum number of letters provided for your given name (first and middle) on the U.S. passport card is 24 characters. The 24 characters may be shortened due to printing restrictions. If both your given names are more than 24 characters, you must shorten one of your given names you list on item 1 of this form.

U.S. passports, either in book or card format, are only issued to U.S. citizens or non-citizen U.S. nationals. Each person must obtain his or her own U.S. passport book or U.S. passport card. The passport card is a U.S. passport issued in card format. Like the traditional U.S. passport book, it reflects the bearer's origin, identity, and nationality, and is subject to existing passport laws and regulations. Unlike the U.S. passport book, the U.S. passport card is valid only for entry at land border crossings and sea ports of entry when traveling from Canada, Mexico, the Caribbean, and Bermuda. The U.S. passport card is not valid for international air travel.

ELECTRONIC PASSPORT STATEMENT

The U.S. Department of State now issues an "Electronic Passport" book, which contains an embedded electronic chip. The electronic passport book continues to be proof of the bearer's U.S. citizenship/nationality and identity, and looks and functions in the same way as a passport without a chip. The addition of an electronic chip in the back cover enables the passport book to carry a duplicate electronic copy of all information from the data page. The electronic passport book is usable at all ports-of-entry, including those that do not yet have electronic chip readers.

Use of the electronic format provides the traveler the additional security protections inherent in chip technology. Moreover, when used at ports-of-entry equipped with electronic chip readers, the electronic passport book provides for faster clearance through some of the port-of-entry processes.

The electronic passport book does not require special handling or treatment, but like previous versions should be protected from extreme heat, bending, and from immersion in water. The electronic chip must be read using specially formatted readers, which protects the data on the chip from unauthorized reading.

The cover of the electronic passport book is printed with a special symbol representing the embedded chip. The symbol port-of-entry areas where the electronic passport book can be read.

File Characteristics

Fact Name Description
Application Purpose This form is used by U.S. citizens applying for their first passport, replacing one that's been lost, stolen, or severely damaged, changing a name that hasn't been legally changed on a current passport, or applying for a child under age 16.
Where to Obtain The DS-11 form can be filled out online or downloaded from the U.S. Department of State website. Paper copies are available at U.S. post offices or passport acceptance facilities.
Submission Method This form must be submitted in person at an authorized passport acceptance facility or a U.S. embassy or consulate if the applicant is residing abroad.
Governing Law Passport issuance is governed by Title 22 of the Code of Federal Regulations, Part 51. This ensures uniformity in the process for all U.S. citizens.

Steps to Writing DS-11 U.S. Passport application

Applying for a U.S. passport is a straightforward process that begins with completing the DS-11 application form. The form is required for first-time applicants, minors, or those not eligible to use the simpler renewal form (DS-82). Filling out the DS-11 correctly is critical for ensuring that the passport processing proceeds without delay. The steps outlined below guide applicants through the process of completing the form, starting from gathering personal information to final submission. Applicants should pay close attention to each step, ensuring accuracy and completeness to avoid any potential setbacks.

  1. Locate the DS-11 form online at the Department of State website or obtain a physical copy at a local passport acceptance facility or agency.
  2. Do not sign the form until instructed to do so by an authorized agent at the passport acceptance facility.
  3. Fill in your full name, date of birth, and place of birth as it appears on your birth certificate or proof of citizenship document.
  4. Provide contact information, including a permanent address, phone number, and email address, to ensure the State Department can reach you if needed.
  5. Enter your parental information, including the full names, dates, and places of birth of your parents. This information is required regardless of the applicant's age.
  6. If applicable, include your spouse's or former spouse's information, such as full name, date of birth, marriage, or divorce dates.
  7. Include details about any previous U.S. passports, including the book number, issue date, and whether it was lost, stolen, or damaged.
  8. List your employment or school information, including the name, address, and telephone number of your current or most recent employer or school.
  9. Provide an emergency contact, detailing their full name, relationship to you, phone number, and address.
  10. If the passport is for a minor, ensure that both parents or legal guardians provide their consent and sign in the presence of a passport acceptance agent.
  11. Attach a passport photo to the designated area on the form. Ensure the photo meets the current U.S. passport photo requirements.
  12. Review the form thoroughly for any errors or omissions. Mistakes can delay the processing of your application.
  13. Gather proof of U.S. citizenship, a valid form of photo identification, and the appropriate passport fee. These will need to be submitted along with your completed DS-11 form.
  14. Finally, take the completed DS-11 form and all supporting documents to a passport acceptance facility or agency. Here, you will take an oath and officially sign the application in the presence of an authorized agent.

Completing the DS-11 form is the first step in obtaining a U.S. passport. By following these steps closely, applicants can ensure their application is processed efficiently, bringing them one step closer to their travels. Remember, accuracy and honesty in providing the requested information are vital, as discrepancies can lead to delays or denial of the passport. Once submitted, applicants can look forward to receiving their passport and embarking on their adventures abroad.

Important Details about DS-11 U.S. Passport application

What is the DS-11 U.S. Passport Application Form?

The DS-11 U.S. Passport Application Form is used by individuals applying for a United States passport for the first time. This form requires applicants to provide personal information, citizenship evidence, a photo, and other documentation to obtain a U.S. passport.

Who needs to complete the DS-11 form?

Individuals applying for their first U.S. passport, children under 16 years old, and those who cannot legally use their previous passport must complete the DS-11 form. It's also necessary for applicants whose previous passport was issued more than 15 years ago, was lost, stolen, or damaged, or those who have had a name change without legal documentation.

Can I submit the DS-11 form online?

No, the DS-11 form cannot be submitted online. Applicants must print the completed form and submit it in person at an authorized Passport Acceptance Facility or U.S. Embassy or Consulate, especially for those applying for their first passport or for children under the age of 16.

What documents are required along with the DS-11 form?

When submitting the DS-11 form, you must also provide proof of U.S. citizenship, a valid form of photo identification, one passport photo, and the applicable fees. If the name on your documents is different from that on your birth certificate, legal proof of the name change is also required.

How much does it cost to submit the DS-11 form?

The cost to submit the DS-11 form varies based on the applicant's age and the type of passport service requested. There are separate fees for the application and the execution service, which are payable to the U.S. Department of State and the acceptance facility respectively. Fee information is updated regularly and can be found on the U.S. Department of State's official website.

What is the processing time for a passport application using the DS-11?

The processing time for a passport application using the DS-11 form depends on the current demand and can vary. Generally, routine service can take between 8 to 11 weeks, whereas expedited service, available for an additional fee, may reduce this time to 5 to 7 weeks. Up-to-date processing times are available on the U.S. Department of State's website.

Common mistakes

When applying for a U.S. passport using the DS-11 application form, many individuals inadvertently make mistakes. These common errors can delay the processing of the application, causing unnecessary stress and potentially impacting travel plans. Being aware of these pitfalls is the first step towards ensuring a smooth application process.

One frequent mistake is not using the correct form. The DS-11 form is specifically for first-time applicants, minors under the age of 16, or applicants who cannot renew their passport by mail. Sometimes, individuals mistakenly use this form when they are eligible to use the renewal form, DS-82, leading to processing delays.

Another common error is providing incorrect documentation. For a DS-11 application, the required documentation includes proof of U.S. citizenship and a valid form of photo identification. Applicants often submit copies instead of original documents or fail to provide the necessary documents altogether. This oversight can halt the application process.

Entering incorrect personal details can also lead to rejection or delays. Small discrepancies in the information provided, such as misspellings of names, incorrect dates of birth, or mismatched information with the provided identification, can flag the application for further review.

Many applicants forget to sign their application forms or, in the case of minors, do not have the necessary signatures from their parents or legal guardians. This oversight is a critical error, as unsigned applications are considered incomplete and are not processed.

Choosing the wrong passport book type is yet another mistake. The U.S. Department of State offers a standard passport book and a passport card. Applicants often confuse the two, not understanding that the passport card is only valid for land and sea travel between the U.S. and Canada, Mexico, the Caribbean, and Bermuda. This limitation can lead to unexpected travel issues.

Inadequate payment is a common error that complicates the application process. The total fee depends on factors like the applicant's age, the type of passport requested, and if expedited service is needed. Sometimes, applicants miscalculate the total cost or provide payment in an unacceptable form, causing delays.

Last, a pervasive issue is the quality of the passport photo. The U.S. Department of State has strict requirements for passport photos, including size, background color, and the subject's pose. Photos that do not meet these criteria will be rejected, necessitating new photos and further delaying the application process.

Understanding and avoiding these common mistakes can help ensure the application for a U.S. passport goes as smoothly as possible. Taking the time to review the requirements, double-check submissions, and thoroughly prepare all documents and photos can significantly expedite the application process.

Documents used along the form

Applying for a U.S. passport involves more than just filling out the DS-11 application form. Various forms and documents are often required to support your application, each serving a specific purpose in the process. From proving your identity to ensuring your eligibility, these supplementary materials play a crucial role. Let's take a look at nine common forms and documents that are typically used alongside the DS-11 form.

  • Proof of U.S. Citizenship: This could be a birth certificate for those born in the United States or a naturalization certificate for citizens born abroad.
  • Proof of Identity: A valid driver's license, government employee ID, or military ID can serve as proof of identity.
  • DS-64: A Statement Regarding a Lost or Stolen Passport, used if your current passport is not available.
  • DS-3053: Statement of Consent, required if one parent is applying for a passport for a child under 16 and the other parent is not present.
  • DS-5525: Statement of Exigent/Special Family Circumstances, for cases where one parent cannot give consent for a child's passport application due to special circumstances.
  • Passport Photo: A recent color photo meeting specific size and quality requirements.
  • Marriage Certificate or Divorce Decree: Needed if your current name is different from that on your proof of citizenship due to marriage or divorce.
  • Flight Itinerary or Travel Plans: May be requested to prove imminent travel if expedited service is required.
  • Fee Payment: Proof of payment for the passport application fee, which varies depending on the type of passport service requested.

In the journey to obtain a passport, each document serves as a stepping stone towards verifying your identity and citizenship, ensuring all legal criteria are met. Whether it's a first-time application or a renewal, understanding the importance of each supplementary document can streamline the process, making it smoother and more efficient. Being prepared with the right documents can significantly expedite your application process.

Similar forms

The DS-82 U.S. Passport Renewal Application is closely related to the DS-11 form, primarily used for adults looking to renew their passport without going in person to a passport acceptance facility. Like the DS-11, it requires personal information, proof of U.S. citizenship, and a recent photo, but it caters to a renewal process, often simpler and more streamlined since the applicant’s identity and citizenship have already been established.

Another document that shares similarities with the DS-11 form is the Form I-9, Employment Eligibility Verification. This form is used by employers to verify an employee's identity and to establish that the worker is eligible to accept employment in the United States. Both forms require detailed personal information and are critical in verifying identity, although for different purposes — one for international travel and the other for employment eligibility.

The Form I-130, Petition for Alien Relative, issued by U.S. Citizenship and Immigration Services (USCIS), is akin to the DS-11 in that it also involves extensive personal information collection, including biographic details for the purpose of establishing relationships. The I-130 is used to prove a valid family relationship between a U.S. citizen or lawful permanent resident and a foreign relative, underscoring the government’s interest in individual identities and relationships similar to the passport application process.

The Form N-400, Application for Naturalization, resembles the DS-11 as it is another crucial form used by individuals seeking a change in their legal status; in this case, applying for U.S. citizenship through naturalization. It requires extensive personal, familial, and residency information, along with a commitment to adhere to U.S. laws, much like the DS-11 demands proof of identity and citizenship.

The Social Security card application (Form SS-5) bears similarities to the DS-11 form in that both are federal applications that require proof of identity, age, and U.S. citizenship or legal status in the United States. Each plays a pivotal role in validating an individual's identification for different but significant aspects of life and legal status in the U.S.

The voter registration application, while more accessible and varied by state, echoes the DS-11's purpose of verifying U.S. citizenship and personal identity, albeit for the purpose of participating in the democratic process. Like the DS-11, it is a gateway for citizens to exercise a fundamental right, with both forms acting as crucial steps in affirming one’s identity and rights within the U.S.

The Selective Service System Registration Form also parallels the DS-11 in its requirement for male U.S. citizens and male immigrants residing in the U.S. to register within a certain age range. This form, similar to the DS-11, gathers personal information and affirms the registrant’s availability for service, thereby playing a role in national defense preparedness.

The driver's license application process in various states shares commonalities with the DS-11 form, as both require proof of identity, a photograph that meets specific criteria, and in many cases, proof of legal presence in the U.S. While one facilitates international travel, the other authorizes the privilege to drive, both crucial for personal independence and identification.

The Global Entry application managed by U.S. Customs and Border Protection resembles the DS-11 in its aim to expedite traveler screening through customs. Both forms require detailed personal information and background checks. However, Global Entry is specifically for frequent international travelers who opt into this program for smoother passage through customs and immigration.

Last but not least, the application for a marriage license, while more localized and varying by jurisdiction, reminds us of the DS-11 form as it involves the collection of personal details and proof of identity to establish the legal eligibility for marriage. Both documents are foundational in their respective areas, enabling individuals to move forward with significant life events under U.S. law.

Dos and Don'ts

Filling out the DS-11 U.S. Passport application form is a crucial step in applying for a U.S. passport for the first time. To ensure the process goes smoothly, there are several do's and don'ts applicants should keep in mind:

Do:

  1. Use black ink: When filling out the form, always use black ink to ensure that all the information is clearly readable.
  2. Complete all sections: Make sure every section of the form is filled out. If a section does not apply to you, write "N/A" (not applicable) instead of leaving it blank.
  3. Sign the form in front of an authorized agent: Do not sign the DS-11 form until instructed to do so by an authorized agent at the passport acceptance facility or U.S. Embassy/Consulate.
  4. Provide truthful information: All the information you provide on the DS-11 form must be accurate and true. Providing false information can lead to serious consequences, including denial of passport, penalties, or prosecution.
  5. Use the correct form: Ensure you are using the DS-11 form, which is specifically for first-time applicants, minors, or individuals not eligible to renew their passport.
  6. Review your application: Double-check your application for any errors or typos before submitting it to prevent delays in processing.

Don't:

  1. Don't use correction fluid or tape: If you make a mistake, start with a new form. Using correction fluid or tape can result in your application being rejected.
  2. Don't leave sections blank: As mentioned, instead of leaving sections blank, fill in "N/A" where applicable to indicate that the question does not apply to you.
  3. Don't guess: If you are unsure about how to answer a question, it's better to seek clarification than to guess and risk providing incorrect information.
  4. Don't fold or damage the form: Keep the form flat and in good condition. Creases or damages can make it difficult for scanners to read the form, possibly leading to processing delays.
  5. Don't use nicknames or initials: Always use your full legal name as it appears on your birth certificate or legal documents. Nicknames or initials can cause issues in verifying your identity.
  6. Don't attach documents with staples or paper clips: When submitting your form, ensure that no staples or paper clips are attached, as they can obstruct the scanning process.

Misconceptions

Navigating the process of applying for a U.S. passport for the first time can be daunting, and there are a few widespread misconceptions about the DS-11 Application Form for a U.S. Passport that applicants often encounter. Understanding these misconceptions can streamline the process, making it less stressful and more efficient for applicants. Below, we'll address and clarify four common misunderstandings.

  • Electronic submission is available for the DS-11 form. One of the biggest misconceptions is that you can complete and submit the DS-11 form online. Unfortunately, this is not the case. Although you can fill out the form online, it must be printed and submitted in person at a passport acceptance facility or agency. This requirement ensures that all necessary original documents and identification can be presented and verified by an authorized agent.
  • No need for a personal appearance if you complete the form online. Even if applicants complete the DS-11 form online, they must appear in person to submit their application if they are applying for their first U.S. passport, are a minor under the age of 16, or do not meet the requirements to renew their passport by mail. This misconception often causes confusion, but the in-person requirement is crucial for verifying identity and ensuring the integrity of the passport issuance process.
  • A birth certificate is always sufficient proof of U.S. citizenship. While a birth certificate is a common and often acceptable form of evidence of U.S. citizenship, it is not universally sufficient. In some cases, additional documentation may be required. This situation may arise if the birth certificate does not meet all statutory and regulatory requirements, such as not listing both parents' names or not having the appropriate seal. Other forms of proof, such as a Consular Report of Birth Abroad, Naturalization Certificate, or Certificate of Citizenship, may also be used.
  • Any passport photo is acceptable as long as it is recent. The U.S. Department of State has strict requirements for passport photos. Not just any recent photo is acceptable. The photo must meet specific criteria, including size (2 inches by 2 inches), background color (plain white or off-white), and the applicant's pose and expression (neutral facial expression, directly facing the camera). Photos that do not meet these requirements will be rejected, potentially delaying the passport application process.

Understanding these misconceptions and preparing accordingly can help ensure a smoother and more efficient passport application process. Applicants are encouraged to review all instructions and requirements carefully and to seek clarification when necessary to avoid delays or additional trips to the passport acceptance facility.

Key takeaways

Applying for a U.S. passport involves a detailed process, of which the DS-11 form is a critical part. Required for first-time applicants, minors, or those not eligible to use the renewal form DS-82, the DS-11 form captures essential information vital for passport issuance. Here are key takeaways to guide you through the process:

  • Use Black Ink: When filling out the DS-11 form, it's essential to use black ink. This requirement ensures the form is legible and meets the standards for digital scanning and processing.
  • Complete the Form Before Your Appointment: Although you must submit the DS-11 form in person, you should fill it out beforehand. This saves time and reduces the potential for errors made in a hurry.
  • Do Not Sign Until Instructed: Unlike many other forms, you should not sign the DS-11 prior to your appointment. Your signature needs to be witnessed by an authorized agent at the passport acceptance facility or the U.S. Embassy or Consulate, if applying from abroad.
  • Accuracy is Key: Ensure all information provided on the DS-11 form is accurate and matches your birth certificate, Social Security card, and other identity documents. Discrepancies can result in processing delays or denial of your passport application.
  • Provide a Social Security Number: If you have a Social Security number, it must be included on your DS-11 form. Failing to provide this can result in significant delays and complications, including denial of your application.
  • Proof of U.S. Citizenship: Along with your DS-11 form, you must submit evidence of U.S. citizenship. This could be your birth certificate, a previous U.S. passport, or other acceptable documents.
  • Recent Photo Required: A recent color photo is necessary when submitting your DS-11 form. The photo must meet specific guidelines—such as being 2x2 inches, taken within the last six months, and with a neutral facial expression.
  • Fees: Be prepared to pay the appropriate application fee. The fee structure varies depending on if you are applying for a passport book, card, or both, and your age. Check the most current fees on the U.S. Department of State's website to ensure you’re prepared.
  • Tracking Your Application: After submitting your DS-11 form and the necessary documents, you can track the status of your application online. This feature allows you to stay informed about the progress of your passport processing.

Navigating the DS-11 form is just one step in the journey to obtaining or renewing your passport but understanding these key aspects can smooth the path. Always refer to the latest instructions from the U.S. Department of State to ensure compliance with current requirements.
